We participate with many different insurance plans and will file your insurance for reimbursement.  It is your responsibility to know the details of your health insurance plan including referrals, co-payments and percentage of coverage.

We require your co-payment at the time of visit.  In addition to cash and checks, we also accept VISA, MasterCard and American Express.  Please call our billing/insurance department at (404) 303-7520 if you have questions about your insurance. We have insurance representatives available to answer your insurance questions, determine which services your insurance covers and assist you with payment options.

Obstetrical and gynecological patients needing referrals to our practice may call their physician’s office.

Billing for Laboratory Procedures

All laboratory procedures are billed separately from your physician appointment.  If you have questions about your laboratory bill, please call:

If you receive Genetic testing, you could also receive a bill from one of the other entities listed above.  If you receive a bill from one of the labs listed above, please call their listed number for your questions.

• Kaiser Multi Choice- We are no longer contracted with Kaiser Multi Choice after 3/1/2009 through the Kaiser Network.   However, Multi Choice patients may be seen under the PHCS network if they have the option to see PHCS contracted physicians.  Patient should verify their network and their benefits through Kaiser.

Decoding your coverage:

• Deductibles are paid out of pocket and must be satisfied each year before insurance pays.  Each family member usually has a separate deductible.

• Co-Payments are out of pocket flat fees charged each time you visit the doctor or use any medical service.

• Co-insurance requires you to pay a percentage of your visit fee.

• Maximum out of pocket expense is the most you’ll have to pay before your insurance begins to pay 100%.
